Lloyd Williams QC and Christian J Howells represented the mother of a teenage boy who took his own life at school. During the inquest, they sought to establish that there had been a failure by both Social Services and the Child and Adolescent Mental Health Services to ensure that DB received the mental health treatment he required. In his narrative verdict, the Deputy Coroner found that there was a failure by Social Services to refer DB to CAMHS and a failure to provide relevant information when inadequate referrals were made by the GP. He found that the outcome for DB may have been different if the referral had been made.
